Family of 3rd preemie to die of overdose speaks
Three Indiana infants die after being given adult size doses of a blood thinner called heparin. Heparin is usually given to premature babies when they are born. A grandmother of a 5 day old premature infant said that the infant had been breathing on her own before she received the drug. The pharmacy technician accidentally stored adult doses in the neonatal unit. The hospital will offer to pay for family counseling and provide restitution.

I think that not only should the pharmacy technician be held accountable for the deaths but so should the nurses in the NICU who administered the drug. The adult doseage comes in a different colored vial from the infant version, so why didn't the nurses in the NICU notice that the colors on the vial weren't normal?
I think that the pharmicist that processed the prescription and the nurse that injected the child should be held accountable. It is very common for premie children to be given blood thinner and the nurse should have noticed the difference in the bottle.
